Problem

Post-upgrade portal improvement depends on collecting feedback in a form that is specific enough to be measurable but broad enough to capture multiple dimensions of usability.

What I Did

Designed the questionnaire structure, grouped the topics into coherent sections, and translated portal concerns into measurable prompts and free-form response opportunities.

Impact

The artifact shows the research instrument behind later usability analysis and provides evidence of how feedback collection was structured before interpretation.

Lessons

Good survey work is design work. The quality of the results depends heavily on how clearly the questions frame the experience being measured. This artifact is the survey form itself rather than the later results deck. It shows how portal feedback was operationalized into structured questions spanning presentation, content relevance, navigation, social behavior and integration needs.
